PATIENT INFORMATION: Day of Your Procedure

• Please arrive one hour before your procedure is scheduled.
• Please wear comfortable, loose fitting clothing and leave all valuables at home.
• Do not wear any make-up, jewelry or contact lenses. Eyeglasses and hearing aides are fine.
• Please bring the following items with you on the day of your procedure:

• Proper identification
• Insurance identification card
• Any medical forms provided to you by your physician
• A list of medications that you are currently taking
• A list of all your allergies
• Any X-rays or other diagnostic images

• Payment of your procedure is due at the time of your appointment at Linden Crest Surgery Center. If you have any questions regarding your procedure fee, please contact us before your appointment.
